Referral Process for Allied Therapists to Pleasure Heals Services

To refer a client to Pleasure Heals, please follow these steps:

Client Intake Form
Fill out the Pleasure Heals Client Intake Form with the client’s details, including their name, contact information, and a brief overview of their needs.

Initial Contact
Introduce yourself, your role, and the reason for your referral. You can choose to schedule a Zoom call for further discussion using our Calendly link.

Client Consent
Obtain the client’s consent to be contacted and ensure authorization to share their information. Please use the Client Consent Form and the Confidentiality Form.

Outreach to Client
Reach out to the client via their preferred communication method (phone or email) to initiate the process.

Scheduling & Initial Assessment
Offer to schedule a consultation or assessment at the client's convenience. During the initial meeting, assess their needs, goals, and concerns.

Treatment Plan & Agreement
Develop a personalized treatment plan based on the assessment. Discuss the plan with the client and obtain their informed consent, including details on confidentiality and treatment goals.

Ongoing Communication & Support
Maintain regular follow-up with the client and allied therapist to monitor progress and adjust the treatment plan as necessary. Ensure all feedback is addressed and keep accurate records of communications, assessments, and treatments.

Review & Updates
Periodically review the client's records and treatment plan to ensure they remain effective and aligned with their goals.

This process ensures a smooth transition and effective management of client referrals, with flexibility and support tailored to the individual needs of each allied therapist’s practice.
